General Information about the Passenger Locator Form to Spain

The Spanish Health Control Form or FCS (Formulario de Control Sanitario) is a digital declaration that contains all crucial details of a traveler’s health status as well as some of their travel-related information.
Using the SpTH form and the details provided in it, the Spanish authorities can easily trace all the persons entering Spain and notify them in case of the potential risk of COVID-19 infection.

Who must complete the Spanish PLF?

All travelers coming to Spain by sea or air, regardless of age, travel purpose, or nationality, are required to complete the SpTH form.
Transit passengers who only plan to stop in Spain before reaching their final destination are also obliged to complete the form in advance of their trip.

Children are not exempt from the obligation of completing the Spanish Passenger Locator Form either. The health forms should be submitted on their behalf by parents or legal guardians. Underage travelers cannot be added to the health declaration of an adult traveler.

Moreover, the PLF to Spain is not transferable, which means that it must be completed for every single trip separately.


When should I submit the Spanish PLF?

You must submit the form within 48 hours before the planned arrival time in Spain. You cannot complete the Spanish PLF earlier since all the data entered must be valid and correct. Moreover, you will need to enter a seat number which is possible to know only after making a check-in for your planned flight.
When completing the form, do not forget to double-check all the information entered. Any mistake in your data may result in some problematic issues during onboarding or crossing the Spanish border.

How to complete the Spanish SpTH Form?

Before filling out the form, prepare your passport or any other equivalent travel document and certificate of vaccination/recovery/negative COVID-19 test. Make sure you have a working electronic device with a stable Internet connection too.

To successfully submit the Spanish PLF, you need to follow these steps:

  1. Fill out the online application form: provide all the necessary data. Check it twice and make sure there is no single mistake.
  2. Cover the service fee: choose one from the available methods of online payment and pay the fee of 34 Euro.
  3. Go to your email address: once your data is submitted, you will receive a confirmation email in a PDF format containing your individual QR code.

Make sure to save its digital or printed copy and carry it throughout your entire trip!


What else do I need to enter Spain?

All passengers arriving from a country or area classified as a coronavirus-risk must submit either:

  • a coronavirus vaccination proof;
  • certificate of recovery or;
  • a negative test report.

All travelers must be prepared to present the SpTH QR code along with one of the certificates (vaccination/recovery/negative test) upon entering Spain.
Children younger than 12 years are exempt from the obligation to provide one of the above-listed documents. Nevertheless, they still must submit the Spanish PLF.
